Rates & Payment

  • Phone Consultation - Free (15 minutes)

  • Individual Counseling (LPC/LCSW) - $125 (50-60 minutes)

  • Individual Counseling (LPC-A/LMSW) - $90 (50-60 minutes)

  • Pre-Martial Counseling - $150 (50-60 minutes)

  • Couples Counseling (LPC/LCSW) - $175 (60-70 minutes)

  • Couples Counseling (LPC-A/LMSW) - $120 (60-70 minutes)

Credit/Debit Card
Personal Check

Payment is due at the time services are provided.

All returned checks or credit card payments are subject to a return charge.

Insurance

Reviive makes every effort to work with insurance panels that are more respectful of the mental health process.  Currently Reviive services clients with Blue Cross Blue Shield, Superior/Ambetter Healthcare, United Healthcare, Aetna, Cigna/Evernorth, Lyra Health, and ComPsych EAP. 

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

We’d recommend asking these questions to your insurance provider to help determine your benefits:

  • Does my health insurance plan include mental health benefits?

  • Do I have a deductible? If so, what is it and have I met it yet?

    • Please note, if you have a deductible, you will be charged the Reviive Therapy private pay rate of service for the service you are receiving until you have met your deductible.

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • Do I need written approval from my primary care physician in order for services to be covered?
